چکیده

Objectives Taking into account the importance of early identification and intervention in patients with SSh in relation to prognosis, we studied few biomarkers, measured within 24 hours from the onset of SSh, along with clinical score to compare their prognostic value for SSh and evaluate their The purpose of the study was to ascertain the prognostic value and evaluate their usefulness. Methods We conducted a prospective cohort study of 100 consecutive critically-ill adult ICU patients on first-time admission with SSh according to the Surviving Sepsis Campaign during one year. Demographic data, severity score (APACHEII and SOFA) and laboratory data were recorded. The program used for the data processing and statistical analysis was SPSS 15.0

Early versus delayed administration of norepinephrine in patients with septic shock

INTRODUCTION This study investigated the incidence of delayed norepinephrine administration following the onset of septic shock and its effect on hospital mortality. METHODS We conducted a retrospective cohort study using data from 213 adult septic shock patients treated at two general surgical intensive care units of a tertiary care hospital over a two year period. The primary outcome was 28...

Septic shock with no diagnosis at 24 hours: a pragmatic multicenter prospective cohort study

BACKGROUND The lack of a patent source of infection after 24 hours of management of shock considered septic is a common and disturbing scenario. We aimed to determine the prevalence and the causes of shock with no diagnosis 24 hours after its onset, and to compare the outcomes of patients with early-confirmed septic shock to those of others. METHODS We conducted a pragmatic, prospective, mult...

Earliest Bedside Assessment of Hemodynamic Parameters and Cardiac Biomarkers: Their Role as Predictors of Adverse Outcome in Patients with Septic Shock

BACKGROUND Early assessment and aggressive hemodynamic treatment have been shown to increase the survival of patients in septic shock. Current and past sepsis guidelines recommend a resuscitation protocol including central venous pressure (CVP), mean arterial blood pressure (MAP), urine output and central venous oxygen saturation (ScvO2) for resuscitation within the first six hours. Currently, ...

Early initiation of low-dose corticosteroid therapy in the management of septic shock: a retrospective observational study

INTRODUCTION The use of low-dose steroid therapy in the management of septic shock has been extensively studied. However, the association between the timing of low-dose steroid therapy and the outcome has not been evaluated. Therefore, we evaluated whether early initiation of low-dose steroid therapy is associated with mortality in patients with septic shock. METHODS We retrospectively analyz...

Caring for the critically ill patient. High-dose antithrombin III in severe sepsis: a randomized controlled trial.

CONTEXT Activation of the coagulation system and depletion of endogenous anticoagulants are frequently found in patients with severe sepsis and septic shock. Diffuse microthrombus formation may induce organ dysfunction and lead to excess mortality in septic shock. Antithrombin III may provide protection from multiorgan failure and improve survival in severely ill patients. OBJECTIVE To determ...
